We are not currently accepting fellowship applications.

 

Prospective Fellows

Prior to submitting your application:

  1. Read Benefits and Expectations for Fellows
  2. Confirm your availability to participate in a two-day Onboarding Training. 

By application deadline:

  1. Complete the online application             
  2. Obtain a letter of support from your employer. (Sample "Employer Letter of Support")
  3. Participate in a brief phone call with the Champion Provider Fellowship liaison within your local health department*

Local Health Departments

Prior to submitting your application:

  1. Read Benefits and Expectations for Local Health Departments 
  2. Confirm your availability to participate in a two-day Onboarding Training. 
  3. Identify your county's primary and secondary liaisons for Fellowship participant(s)
  4. Identify healthcare provider(s) you would like to nominate in your county for Fellowship

By application deadline:

  1. Complete the online application           
  2. Obtain a letter of support from your local health officer (LHO). (Sample "LHO Letter of Support")
  3. Participate in a brief phone call with the Champion Provider Fellow applicant in your county*
